carvedilol

Mechanism

Additive hypotension and sedation. Carvedilol's α/β-blockade causes fatigue and dizziness on its own; alprazolam adds GABAergic CNS depression.

Symptoms

Sedation, fatigue, slowed reactions, impaired coordination. In older patients: fall and fracture risk. Driving and operating machinery: accident risk. Postural dizziness, syncope, fatigue. Older patients: fall and fracture risk.

Management

On starting the combination, begin alprazolam at the minimum dose (0.25 mg at bedtime). In older patients, cap at 0.5 mg/day. Check standing/sitting BP at one week. With pronounced sedation, consider non-drug options for anxiety (CBT, breathing exercises) or switch to buspirone (no sedation).

Mechanism

Additive bradycardia and QT effect from amiodarone, on top of carvedilol's negative chronotropy. Often used in heart failure with reduced ejection fraction and atrial fibrillation — an acceptable combination.

Symptoms

Bradycardia, dizziness, fatigue, near-syncope. QT prolongation on ECG. Dizziness, syncope, palpitations. Rarely: polymorphic ventricular tachycardia (torsades de pointes). Risk is higher with hypokalaemia, hypomagnesaemia, bradycardia, and ischaemic heart disease.

Management

Start carvedilol at minimum dose (3.125 mg twice daily) with up-titration every 2 weeks. Check ECG and heart rate at 1 and 4 weeks. If heart rate falls below 50 bpm or 2nd/3rd-degree AV block develops, adjust both. Keep potassium and magnesium in the upper half of normal range.

Mechanism

TCA blocks α1-adrenoceptors — additive orthostatic hypotension with carvedilol (α/β-blocker). Amitriptyline's additive bradycardia and QT effect intensify as heart rate drops with carvedilol.

Symptoms

Postural dizziness, syncope, fatigue. Older patients: fall and fracture risk.

Management

On starting the combination, begin amitriptyline at 10 mg at bedtime. In older patients, do not exceed 25 mg/day. Check standing/sitting BP, heart rate, and ECG at 2 weeks. For neuropathic pain, alternatives: gabapentin or pregabalin. For depression: sertraline or escitalopram (minimal cardiovascular effect).

Mechanism

Additive hypotension and peripheral oedema. Amlodipine causes ankle oedema; the beta-blocker (carvedilol) may potentiate fluid retention.

Symptoms

Lower-leg and ankle oedema, weight gain. Hypotension and dizziness on standing. Worsening dyspnoea in heart failure patients.

Management

Standard combination in ischaemic heart disease with hypertension. In heart failure with reduced ejection fraction, titrate carefully and monitor weight and dyspnoea. For marked ankle oedema: switch to lercanidipine (less oedema among dihydropyridines).

Mechanism

Systemic budesonide retains sodium and water, blunting the beta-blocker's antihypertensive effect. Inhaled budesonide does not clinically affect BP.

Symptoms

On long-term systemic therapy: return or rise of blood pressure, leg oedema.

Management

For short systemic budesonide courses, no specific adjustment needed. For long-term therapy, check BP weekly in the first month. If BP rises, increase carvedilol or add an ACE inhibitor. Inhaled budesonide with carvedilol is unproblematic.

Mechanism

Beta-blockers mask adrenergic hypoglycaemia symptoms (tachycardia, tremor, sweating). In diabetics on insulin, the risk of severe unrecognised hypoglycaemia rises.

Symptoms

Confusion, vision changes, loss of contact, seizures. Tachycardia, tremor, and sweating are suppressed — the patient misses the hypoglycaemia.

Management

Prescribe carvedilol cautiously in diabetes. In heart failure with reduced ejection fraction, carvedilol remains first-line (improves survival) — teach the patient to recognise hypoglycaemia by any unusual sensation. On insulin, intensify glucose self-monitoring (4 times daily) for the first month. For hypertension without heart failure, alternatives: ACE inhibitor or ARB with a diuretic.
