Inhaled corticosteroid (ICS) plus long-acting β2-agonist (LABA) combination with rapid onset

Budesonide is an inhaled corticosteroid with strong anti-inflammatory action on bronchial mucosa, reducing hyperreactivity and eosinophilic infiltration. Formoterol is a selective β2-agonist with rapid onset (3 minutes) and 12-hour duration. The combination delivers anti-inflammatory control and sustained bronchodilation in one inhaler. Formoterol's fast onset enables both maintenance and reliever use (the MART strategy).
First line
First-line in the MART (maintenance and reliever therapy) strategy from 2024 step 2. As-needed use in place of SABAs reduces exacerbation risk by 26–46% (SYGMA 1, SYGMA 2, Novel START). Standard regimen: 160/4.5 mcg twice daily plus 1 inhalation as needed for symptoms, up to 12 inhalations per day.
Second line
In COPD, used in patients with frequent exacerbations and blood eosinophils above 300 cells/mcL ( 2024). Dose 320/9 mcg twice daily. Reduces exacerbations by 25% but raises pneumonia risk – benefit outweighs risk only in confirmed eosinophilia or eosinophilic COPD phenotype.

Budesonide/formoterol is a fixed combination of an inhaled corticosteroid (ICS) and a long-acting β2-agonist. It is prescribed for asthma and COPD ( 2024, 2024, AEMPS Ficha Técnica). Budesonide/formoterol is used for cold prevention and enhanced breathing outside an asthma or COPD indication. No clinical studies of prophylactic use in this setting exist. The drug causes hoarseness, oropharyngeal candidiasis, and with long-term high-dose use systemic corticosteroid effects (adrenal insufficiency, osteoporosis, cataract). If budesonide/formoterol was prescribed outside confirmed asthma or COPD, consider seeking a second opinion.
