Per 2021, 2020, 2018, azathioprine is a maintenance therapy in moderate-to-severe Crohn disease and ulcerative colitis, often combined with biologics (anti-TNF). Dose 2–2.5 mg/kg/day. Effect develops over 8–16 weeks. Reduces flare frequency, corticosteroid requirements, and hospitalization rates.
